Shared Health Dual Plus and Shared Health Dual Freedom are Dual Eligible Special Needs Plans – types of Medicare Advantage plans for people enrolled in both Medicare and Medicaid. We help coordinate these benefits for our members and provide extra help with things like dental and vision care at no additional cost.

Steps to join our network

Here’s what you’ll need to enroll as a new provider for Shared Health.

1 Work with your health system, hospital or other third-party vendor to complete your enrollment application and begin the credentialing process. If you’re not part of a group contract, contact us, and we’ll help you get started.

2 Enter or update your information in CAQH EnrollHub.

3 Register with Availity.
